If you are eligible for a flu vaccine you may receive a text from Essex County Council offering support.  The text will contain a link to www.essexwelfareservice.org.uk and a phone number. Please be aware that this is not a scam.

If you are due for your Annual Asthma Review please click on 'Asthma Review' under 'Further Information' and complete the questionnaire.

HUB appointments - To cancel a weekend or evening appointment at the Dunmow Clinic please call 07880 398840 or 111 and select option 3.

For EMERGENCIES WHEN THE SURGERY IS CLOSED call 111 or go to 111.nhs.uk (for people aged 5 and over only).  NHS111 is available 24 hours a day, 7 days a week and calls to this service are free from both landlines and mobiles. 

We are in the process of moving our online ordering of repeat prescriptions for patients over 18 years to a new system. Patients will need to attend the surgery with photographic ID (Photo Driving Licence, Passport etc.,) to register for the new service.  When they have registered for the new service patients will also be able to book appointments on-line and view their medication, allergies and immunisations. When you have received your registration details from the surgery please click on the Patient Access button below to access the service.  

Pre-Registration With the Practice

johntaskerIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. You will need to visit the surgery and sign the form to confirm that the details are correct, before your registration can be completed and before you book an appointment or order medication.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to Felstedus. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
